I am building a game in which i have the following flow

• Game Intro
• Game instructions
• Game play
• Game Form
• Game Results

The sequence of events for the game is

• play game intro....
  • There is an age check form to fill out before continuing
• either play the game or view the instructions
  • if view the instructions on game play you go to the game play
• Play the game
  • when the game is over...either you play again or fill out a form
• Fill out form
• After filling out the form you view the high scores

Given the kind of sequence this gmae takes would you recommend me using Screens? Slide? or Slide and Form?
